To improve health and reduce disparities in care, leading companies and healthcare systems are focused on addressing social determinants of health (SDoH), such as socioeconomic status, education, neighborhood and physical environment, employment, and social support networks. Health plans and employers continue to invest millions in initiatives that aim to address the root cause of many of today’s most intransigent healthcare problems including chronic disease, cancer and disparities in mortality and health outcomes.
